The process is very simple. You go through a checkout process at our web-site and during check-out you are asked a series of questions to determine whether you are a suitable candidate for the device.
Once you finish the questionnaire and complete the check-out, we will share, via a HPAA-compliant service, your questionnaire with an independent licensed healthcare provider who will review the answers you provided and, if approved, will issue and signed medical device authorization form which will allow us to ship the device to you.
